Home from Hospital

Should you require some practical help to get home after a stay in hospital,

Aster care (external link) can help with some little changes to help you settle back into everyday life.

Homes from Hospital is a free service, providing practical help before you go home, or the first few weeks you're home.

The care and repair team works together with local authorities as well as the NHS in Somerset.

Specialist caseworkers and Handihelp technicians work with hospitals and patients to make sure your home is safe to return to.

Who can be helped

This service is available to residents in Somerset who:

  • are over the age of 60
  • are 18 and over with a disability
  • are living alone
  • are living with a partner with limited means
  • have no family living nearby

How you can be helped:

  • moving beds or other furniture
  • installing/moving a telephone so its easy to reach
  • setting up food delivery services
  • fitting keys safes
  • organising minor home improvements and adaptations
  • help you sort out what benefits your are entitled to
  • telcare six week free trial
  • home safety checks
